From what I understand of ascite, as a build-up of fluid, then in a clinical examination "Abdomen sous tension" would mean "increased pressure in the abdomen" "build-up of fluid in the abdomen"or more technically "intra-abdominal hypertension". From the patient's point of view, they would have a "distended abdomen" or "swollen abdomen" perhaps feel tightness in the area or bloated.
As for the "ascite reste sous tension", perhaps "ascite pressure remains high" or "continuing intra-abdominal hypertension", but you will know better how to phrase this with the extra context you have in the text.

Abdominal compartment syndrome (ACS) is the end point of a process whereby massive interstitial swelling in the abdomen or rapid development of a space-filling lesion in the abdomen (such as ascites or a hematoma) leads to pathologically increased pressure. This results in so-called intraabdominal hypertension (IAH), causing decreased perfusion of the kidneys and abdominal viscera and possible difficulties with ventilation and maintenance of cardiac output.

Elevated intraabdominal pressure (IAP) has been claimed to play a role in abdominal compartment syndrome. We assessed the correlation between the values of IAP, abdominal perfusion pressure (APP) and clinical scoring systems including SIRS, MODS and APACHE II and the patients’ survival rate in patients admitted to the ICU with acute abdomen due to ileus, intestinal perforation, peritonitis and trauma.
We measured IAP and APP in 50 surgical patients. In this study the IAP was measured in a non-invasive manner via urinary bladder pressure. The APP was calculated as the difference between mean arterial pressure and IAP values.
A significantly higher IAP was found in the non-survivors’ group in comparison with the survivors’ group. On the other hand, the APP inversely correlated with disease severity scores including SIRS, MODS and APACHE II, whereas IAP values did not show any correlation to these clinical parameters. These findings suggest that IAP and APP may be useful tools in the clinical evaluation of patients with acute abdomen.
