Source text - English In 1887 Theodore Mander began building a new house in the country near Wolverhampton. He called it Wightwick Manor.
Theodore was 34 and a partner in a successful family business making varnish and paints. He and his wife - aptly named Flora Paint- had two young children and a third on the way.
Wightwick still feels like the picturesque family home the Manders planned it to be. It's a house you can easily imagine living in - comfortable and inviting, full of interest, with every room different. You can picture curling up in one of its window seats or warming yourself by a log fire in one of the inglenook fireplaces. In the depths of the garden you easily forget you're on the outskirts of a Midlands city.
Theodore built Wightwick in two stages, although he hadn't planned it that way. He commissioned architect Edward Ould to design the house in the Old English style of timber framing, tile-hanging and bright red brick. In 1892-3 they added the east wing with elaborately patterned oak framing and brick chimneys.

Source text - Spanish La paciente Lu Carr, con D.N.I 4866***** está acudiendo a la Clínica
Campos Fisioterapia desde Octubre del año 2017, tras una primera intervención
quirúrgica realizada el 1 de Agosto del 2017, y una segunda, el 31 de Octubre del 2018,
producido por una accidente de tráfico el 21 de Julio del 2017.
A consecuencia de ese accidente, la paciente sufrió una fractura cerrada de meseta
tibial derecha y una rectificación a nivel de la columna cervical (diagnóstico médico).
En la primera consulta de fisioterapia, en la exploración fisioterapéutica pudimos
hallar:
- Dolor anterior y en el compartimento interno de la rodilla (Escala EVA 7).
- Cicatriz ligeramente adherida.
- Temperatura aumentada de la rodilla derecha respecto a la izquierda.
- Fuerza muscular de 3+/5 en musculatura cuadricipital y de 3+/5 en
musculatura isquiotibial (Escala Kendall).
- Hipotonía muscular generalizada en todo el MI derecho.
- Descoordinación entre la musculatura agonista y antagonista del MI
derecho.
- Ligera limitación a la flexión máxima de rodilla.
- Marcha estable con 2 muletas.
- Pérdida de la propiocepción podal.
- Dolor en musculatura cervical y cuello (Escala EVA 6)
- Puntos gatillos en musculatura suboocipital, esplenia, trapecios y angulares.
- Limitación de la movilidad en segmentos cervicales altos y bajos, además de
la charnela cervico-dorsal.
El tratamiento que hemos seguido durante las sesiones hasta la fecha actual ha sido el
siguiente:
- A nivel de la rodilla:
o Terapia manual para adherencias cicatriciales y rótula.
o Movilidad de la rodilla en todos sus rangos articulares.
o Masaje descostracturante para toda la musculatura del MI derecho.
o Terapia miofascial para MI derecho.
o Trabajo de fuerza para musculatura agonista y antagonista de MI
derecho. En cadena cinetica cerrada y abierta.
o Propiocepción para la rodilla derecha en carga y descarga.
o Punción seca para puntos gatillo miofasciales en musculatura
isquiotibial y cuadricipital.
o Indiba para la musculatura isquiotibial, cuadricipital derecha y
compartimento anterior lateral y posterior de la rodilla. Terapia
analgésica y antiinflamatoria.
o Neuromodulación nervio femoral, safeno, obturador, ciático, tibial.
Terapia analgésica y funcional.
o Laser (K-laser) alta potencia para cicatriz y compartimento anterior
de rodilla.
o TENS analgésico.
- A nivel cervical:
o Terapia manual de movilidad de segmentos cervicales altos y bajos.
o Terapia miofascial.
o Punción seca para músculos suboocipitales, esplenios, trapecios,
angulares de la escápula.
o Indiba para toda musculatura de cuello y cervical.
o Neuromodulación en nervio espinal, arnold, dorsal escapular.
o Masaje descontracturante para toda la musculatura.
o Ejercicio activo McKenzie cervical.
Al cabo de unas 3-4 sesiones mensuales los resultados han sido los siguientes :
- El dolor se ha reducido considerablemente (Escala EVA 1).
- Cicatriz y rótula sin adherencias.
- Sin limitaciones a la movilidad de la rodilla.
- Menos tensión en musculatura de MI derecho.
- Aumento de la fuerza en todo su complejo muscular 4+/5 (Escala Kendall).
- Marcha completamente estable sin ningún tipo de ayuda.
- Aumento de la propiocepción podal.
- Ligera mejoría en el dolor cervical-cuello (Escala EVA 3-4).
- Ligera mejoría en la movilidad de los segmentos cervicales altos y bajos.
Por concluyente, visto la valoración final, recomiendo realizar una sesión semanal para
mantener la fuerza y propiocepción de la rodilla derecha. Y, sobre todo, terapia para la
columna cervical, que visto los resultados, hubo menos mejoría en comparación con la
rodilla.

Translation - English The patient Lu Car, with D.N.I 4866***** has been attending the Campos Physiotherapy Clinic since October 2017, after a first surgical intervention performed on August 1, 2017, and a second, on October 31, 2018, caused by a traffic accident on July 21, 2017.
As a result of that accident, the patient suffered a closed fracture of the right tibia and a rectification of the cervical spine (a medical diagnosis).
In the first physiotherapy consultation and in the physiotherapeutic examination we were able to find:
- Anterior pain and pain in the inner compartment of the knee (EVA Scale 7).
- Slightly adhered scar.
- Increased temperature of the right knee with respect to the left.
- Muscular strength of 3+/5 in quadricepital muscles and 3+/5 in
ischiotibial muscle (Kendall Scale).
- Generalized muscular hypotonia throughout the right MI.
- Discoordination between the agonist and antagonist muscles of the right MI.
- Slight limitation to maximum knee flexion.
- Stable gait with 2 crutches.
- Loss of proprioception.
- Pain in cervical and neck muscles (EVA Scale 6).
- Trigger points in suboocipital, splenia, trapezius and angular muscles.
- Limited movement of the upper and lower cervical segments, in addition to the cervical-dorsal hinge.
The treatment that we have followed during the sessions till date is as follows:
- At the knee level:
o Manual therapy for scar and kneecap adhesions.
o Mobility of the knee in all its articular ranges.
o Decostracting massage for all the muscles at the right MI.
o Myofascial therapy for MY right.
o Force training for agonist and antagonist muscles of MI.
In a closed and open cinematic chain.
o Proprioception for the right knee in loading and unloading.
o Dry puncture for myofascial trigger points in hamstring and quadricep muscles.
o Indiba for hamstring, right quadriceps muscles with anterior and posterior knee compartment. Analgesic and anti-inflammatory therapy.
o Neuromodulation femoral nerve, saphenous, obturator, sciatic, tibial. Analgesic and functional therapy.
o High power Laser (K-laser) for scar and anterior compartment on the knee.
o Analgesic TENS.
- At the cervical level:
o Manual mobility therapy of upper and lower cervical segments.
o Myofascial therapy.
o Dry puncture for suboocipital muscles, splenios, trapezius of the scapula.
o Indiba for all neck and cervical muscles.
o Neuromodulation in spinal nerve, arnold, scapular dorsal.
o Decontracting massage for all the muscles.
o Active McKenzie cervical exercise.
After about 3-4 monthly sessions the results have been the following:
- Pain has been considerably reduced (EVA Scale 1).
- Scar and kneecap without adhesions.
- No limitations to the mobility of the knee.
- Less tension in right MI muscle.
- Increased strength throughout the muscle complex 4+/5 (Kendall Scale).
- Complete stable gait without any assistance.
- Increased foot proprioception.
- Slight improvement in cervical-neck pain (EVA Scale 3-4).
- Slight improvement in the mobility of the upper and lower cervical segments.
In conclusion, given the final assessment, I recommend a weekly session to maintain the strength and proprioception of the right knee. And, above all, the use of therapy for the cervical spine, which was seen with less improvement compared to the knee.
