So, is it correct to say either of the following :
"The consulting doctor: I have counseled this patient etc."
OR
"The consulting doctor: I have given this patient advice etc."
or something else?

Ethan- Do not do a word for word translation unless this is a backtranslation and you are checking on the translator.
You have to use the proper terminology for the clinical research field . In order for a subject to participate in a trial/study, the physician who will conduct the study (the investigator) or his assistants (sub-investigators) have to inform and explain to all prospective study participants all the elements of the study (duration, number of participants, possible risks, who to contact in case of injury, etc.) so they can be "informed" and then agree or not to participate by signing or not signing the "Informed Consent Form". The investigator/sub-investigator has also to answer the subjects questions regarding the study and advise them about their queries.
Azmi Jbeili has it right regarding: information/explanation etc. also that in order for a person to sign such a form, the person should understand it first.
basically, it is the physician who comes to the patient with all the information ( since the doctor is recruiting subjects for his/her study) rather than the patient going to the physician for consultation.

The Arabic provided is inaccurate; the terms used are uncommon especially for a consent form which should be written in layman Arabic – MSA not classical Arabic, in addition to spelling error.

However in this context “advice” is inaccurate even in English, it should be “advise”, furthermore in Arabic this concept is always conveyed by using “explanation/instructions/information” , it is a “consent form” after all.

According to Almoheet Arabic dictionary النُّصْحُ =إخلاصُ المَشورة.

I totally agree with the use of "advise".
Means literality “medical consultation”, however, it is a language practice in Arabic [for Israeli Arabs at least] and Hebrew to mean “Medical counselling / consultation”.
I have conducted many cognitive debriefing questionnaires in Israel for Arabs and Jews alike. Every consent form we have used included the term “I have advised/explained the nature of the procedure to this patient ...”, therefore, I am inclined to agree to using your expression
“I have advised this patient regarding the nature of the procedure”
Thanks for the detailed explanation, and the comparison to Hebrew - I do know it's ייעוץ רפואי in Hebrew, so let's see if we can nail the English :)
The spelling error is mine - It was قدمت is the original, but typing Arabic here often requires another four pairs of glasses, and I only wear one :)
I guess the question is what نصائح best means here. Wehr says nothing about "explanation", even though I realize it can work. So -
"The advising doctor : I have advised this patient regarding the nature of the procedure". That sound good to you?, Or rather : "...I have provided the patient with information about...". I'm just not sure the latter is accurate. Thoughts?
