A. Title Page
Some agencies include specific title pages in their application packages that must be used when preparing a proposal. In the absence of a formal title page supplied by the sponsoring agency, it is suggested that the Principal Investigator (PI) follow the sample format that appears in Appendix 4. If the format in Appendix 4 is not used, the title page should, at a minimum, include the following:

1. the title of the project (remember that some agencies limit the number of characters allowed in the title);
2. the full name of the institution - University of Houston;
3. the PI's name, University phone and fax numbers, e-mail address, and University address, including the appropriate 4-digit mail code found after the zip code;
4. the names of any co-PIs and their contact information;
5. the name of the agency to which the proposal is being submitted;
6. the duration of the project;
7. the amount of funding that is being requested;

Formal Proposals Generally Include (in this order):
Cover Sheet/Title Page
This identifies the proposal and includes the following: the title of the proposal; name, title, and address of the person or organization for whom the proposal is written; identifying number (if there is one); date of submission; and name, title, and address of the person or organization submitting the proposal.
