I tried to help all I could so I too am irritated with the uncalled for remark when all everyone was asking for was additional context and I was certainly not the only one asking for context. You only supplied the additional context after much prodding.

I will stay with my initial thoughts and say "Formal (Criminal) Complaint". HTH!
2) It is being presented to the judge during what is presumably a preliminary hearing (it doesn't actually say).

5) The plaintiffs are being accused of Attempted Manslaughter and Illegal Possession and Use of Firearms and Weapons of War.
The following after the phrase previously given deals with the individual suspects the crimes attributed to each one of them and the legal backing for calling what they did a crime.
