Scant context leaves different interpretations. Both 'bed' and 'base' are possible in the scant context provided. They're both used often, referring to different things that could each be expressed in Spanish as 'fondo'. 'Bed' would be the area of tissue around the (single) ulcer in question, which area would also be irritated and possibly contain other ulcers. 'Base' would be the surface of tissue covered by the ulcer, reached for inspection by removing the necrotic & other material of the ulcer, itself. '(Genetic) background' of the ulcer doesn't seem to fit the (again, I say 'scant') context as it appears, but is also possible for 'fondo', in general. Best regards, All!
