Zwrot medical card i inne pochodne z ‘card’ w kontekście ‘karty’ są powszechne w języku medycznym.

Raczej nie - outpatient services card (nie brzmi ogólnie i nie brzmi po angielsku).

Osobiście sugerowałbym tłumaczenie opisowe. Nie spowoduje przekłamania w przekładzie.
