So the idea is "to get an extract" of the "personal card" from the relevant authorities (civil service) and put it in a green map along with the other documents.
This looks to be a Saudi case where they have this "personal card" (bitaqa shakhsiyyah). Note that this is not the same as an ID (bitaqat at-ta'rif).

Sorry, forgot to change the title of my answer. You can use 'obtain an extract' for a more accurate translation, or simply 'provide' for simplicity.
