To simplify the suggested translation, you may use [ معالجہ اور بحالی صحت ] to cover major aspects of "physical medicine and rehabilitation". Word [sehat-صحت] would make the display sign at the door, more sensible and simple to understand for common approach that this room/department is about "recovery of health" without صحت , the word بحالی would be still remain unclear as "recovery" for what?.
Please keep in mind that word بحالی صحت is common major Hospitals, such as Liaqat National Hospital Karachi, Agha Khan Hospital, Civil Hospital and other major Hospitals in other cities of Pakistan.

: Although 'medicine' literally means 'دوا', but considering that it is to be written on a sign, 'طب' is better. In the same context, 'صحت' appears to be extra, but can be tolerated however, making the phrase as 'طبعی طب اور بحالیٔ صحت'

being a native speker of Arabic I would like to clear that علاج=medicine=a substance used in treating disease and معالجہ=medication=the act of medicating. thats why I think there is a slight difference bet. my translation and Zack's I hope i made it cle

I looked up this term and it specifically refers to the field of medicine that deals with physical disability or bodily pain that needs management, and does not include mental rehab.
Thus the word 'jismani' for the term 'physical' is a befitting term...one easily understood. Also, 'ilaaj' used for 'medicine' is a more intuitively understood term than 'tib' which gets a bit abstract for the lay person.

I used 'jismani' to describe the word 'physical' to a lay person as the asker did not find the prior answers meeting that need.
After considering all comments, I think Attiya Khan's suggestion of طبعی معالجہ اور بحالی صحت seems most appropriate description of this field of medicine. Notes or comments can be added as an aside to explain the field to the lay person.
