Educational needs assessment would be quite wrong in my opinion. The session is being used not only to diagnose the patient, but to educate them (and perhaps the doctor too) about their condition.

Surely this is not about educational needs assessment though?
All the references point towards "educational diagnosis".
Also, this is about an 80-year-old patient, not a child or adolescent.

Educational diagnosis is the educational assessment of students in order to establish possible cognitive, emotional, health, perceptual, social and other factors that might be impacting on their academic achievement and school adjustment. Such diagnosis can also be sought where dyslexia, learning difficulties, learning disorders or reading disabilities etc are suspected.

this 80-year-old patient got so worried about his academic achievement that he ended up with decompensated heart failure? Surely makes sense!
