Cardiology Report
This is at the top of this short one-page report, underneath the Dr's name and the department name (Clinical cardiology for adults).
To put "Contract / Meeting" in English would look a bit odd, so can somebody explain to me what exactly this is for, rather than give a literal translation.....? (i.e. I do not need a word-for-word translation without an explanation).

It most probably refers to the contractual relationship between the service user and Casa Națională de Asigurări de Sănătate

: The only thing I, as a native English speaker or 'squeaker', can add is that 1. 'conventie' is a false friend with a convention meeting and 2. the UK lawyers' preference of a translation of a (non-binding) agreement for 'contract' would be tautologous.

It is a false friend, indeed. However, "convenție" seems to refer to an agreement between states, which is not the context here

Absolutely, "policy" is not the equivalent here but it is a type of contract

: AFAIK the contract/agreement between the doctor (or perhaps the cardiology department) and the health insurance authority cannot be referred to as a 'policy'.
