The phrase "efectiv verificat" is a tab in a spreadsheet entitled "Rezultatul inspecţiei privind respectarea cerinţelor legale în materie de gestionare (SMR)". Some other tabs are: "Cerință de verificare SMR", "Număr", "Neglijență", etc.

This sounds like a column to enter whether the preceding information/data given has been checked or verified, eg. data that has been effectively checked.j

For this we would simply indicate at the top of the column whether the information had been "verified" or not with an entry of yes or no (or cross or tick mark). "efectiv" would be assumed in the title.
